ADMISSION

Admission is an important function of the Office of the Registrar.  The office ensures that the new student applicants’ academic records and all other requirements are screened and verified.  Assistance is provided to the applicants from inquiry to entrance examinations, interview and registration.

Admission and Registration

ADMISSION REQUIREMENTS AND PROCEDURE

Selection for admission is based on the Report Card, result of the  entrance examination, deportment and interview.

An applicant must submit a duly accomplished application form signed by his/her parents, and the following documents:

Original and photocopy of Birth Certificate from the  Philippine  Statistics Authority (PSA)

Duly signed Report Card (with passing grade per subject) and transfer certificate from school  previously attended

Certification of Good Moral Character from the Guidance Counselor

Sealed recommendation from the principal or adviser (CSI Recommendation Form)

2 copies of 2×2 picture

For INTERNATIONAL STUDENTS:  9e Visa / Permanent Residence Visa (PR) and ACR Card and scholastic record with English translation. The Special Study Permit (SSP) will be processed at the Bureau of Immigration (BI) if the International student is a holder of tourist visa (9A) and Balikbayan Visa (BB).

A returning student must fulfil the admission requirements for continuing students, that is, report card with passing grades.

A student who took summer classes must submit his/her permanent record, report card, and a copy of recomputed grade/s from the school where he/she enrolled for summer. He/She should pass the validating exam.

Application for Admission Procedure

STEP 1: Filling Out of Form

Fill out the APPLICATION FOR ADMISSION FORM.

STEP 2: Submission of Requirements

Submit the required documents together with the duly accomplished Application for Admission Form.

STEP 3: Exam & Interview

Take the entrance exam on the date specified. An interview will follow after the exam.

STEP 4: Confirmation

Wait for the confirmation from the Office of the Registrar to proceed to Step 5.

STEP 5: Payment of Reservation Fee

Pay the Php1,500 Reservation Fee at the CSI cashier or  via bank deposit  or online transfer at  the following bank accounts. The Reservation Fee is non-refundable and serves as your initial payment for the school year.

If payment is made via bank deposit or online transfer, send a clear copy of the proof of payment to csi.[email protected] and csi.[email protected]. Your message should contain the following details:

STEP 6: Issuance of Official Receipt

Get the official receipt from the CSI cashier. If payment is made via bank deposit or online transfer, get the receipt after receiving the confirmation message from the Finance Office.

Enrollment Procedure

STEP 1: Encoding of Student Data

Carefully encode all student information at the enrollment system unit assigned to generate the Student Registration Form.

STEP 2: Verification

Bring the printed Registration Form to the verification desk for the verifier’s signature.

STEP 3: Assessment of Fees

Proceed to the Assessment Desk for Assessment of Fees.

STEP 4: Payment of Fees

Pay the school fees to the cashier or via bank deposit  or online transfer at the following bank accounts:

STEP 5: Issuance of Official Receipt

Get the official receipt from the CSI cashier. If payment is made via bank deposit or online transfer, get the receipt after receiving the
confirmation message from the Finance Office.

STEP 6: Final Checking

Proceed to the Final Checking Desk for final checking of enrollment papers.  All the documents presented will be stamped ENROLLED and duly signed by the Final Checker.

STEP 7: Issuance of Registration Form

Present the OR to the Registrar’s Office for the issuance of Registration Form and Enrollment Slip/Admit to Class Slip.

 

STEP 8: Issuance of Books

Present the OR to the  book store for release of text books.
